We look forward to meeting you on your first visit to our office. We understand that each patient’s orthodontic treatment is different, and we promise to give you the personal attention you deserve. Your first orthodontic visit will consist of an examination and discussion of potential treatment options. This important visit will address your orthodontic needs to determine if this is the proper time to begin treatment.
New Patient Forms
During the initial examination for each patient, we will address the following questions that cover the basics of orthodontic treatment.
- Is treatment needed now or should treatment be delayed until appropriate growth, tooth eruption or other factors have occurred?
- What treatment procedures will be used to correct the problem?
- Do any teeth need to be removed?
- How long will treatment take?
- How much will it cost?
- What are my payment options?
During your initial exam, Dr. Anderson will explain all aspects of your treatment using photos and X-rays. Our office will provide a complete clinical diagnosis regarding the nature of the problem, goals and fees. We encourage questions from both the patient and the parent, and as such, we will provide ample time for Dr. Anderson to answer your questions.
It is our mission to provide you with high-quality, affordable treatment that fits your budget! We are sensitive to the fact that different people have different needs in fulfilling financial commitments. We offer a variety of payment options to meet your needs, including but not limited to the following:
- Payment in full at the beginning of treatment with a fee discount.
- A payment plan to be extended over a period of months subsequent to an initial down payment.
- A third party lending firm which requires no initial payment and payment plans ranging from 18-60 months with reasonable monthly payments.
We will thoroughly explain each of these options so that we are able to accommodate your needs. For your convenience, our office accepts MasterCard and Visa.
The majority of orthodontic appointments are short in nature and take place every four to eight weeks. There will be a few procedures that will require morning or middle of the day scheduling. We will work hard to accommodate your scheduling requests.
Your scheduled appointment has been specifically reserved for you. We value your time and pride ourselves in our ability to see you at your scheduled appointed time. We are aware that unforeseen events sometimes require missing appointments. We try our best to reschedule appointments at a time that is convenient for our patients.
Our office is committed to helping you maximize your insurance benefits and will help you determine the extent of those benefits. Orthodontic insurance is more often than not a lifetime benefit and is usually separate from dental insurance. Our office will prepare necessary forms to assist you in collecting your benefit. Please remember plans only pay a portion of your fee. We accept most insurance so please call our office if you ever have any doubts or questions. Our courteous staff is always available to assist you with your financial and flex account questions, as well.
